Overview

Carbimazole used in the treatment of hyperthyroidism. It depresses the formation of thyroid hormone. Carbimazole is a pro-drug as after absorption it is converted to the active form, methimazole. Methimazole prevents thyroid peroxidase enzyme from iodinating and coupling the tyrosine residues on thyroglobulin, hence reducing the production of the thyroid hormones T3 and T4 (thyroxine).

Indications

Carbimazole is primarily indicated in conditions like Blocking-replacement, Graves disease, Hyperthyroidism.

Contraindications

Carbimazole is contraindicated in conditions like Nodular goitre,Sub-acute thyroiditis,Post-partum painless thyroiditis.

Side Effects

The severe or irreversible adverse effects of Carbimazole, which give rise to further complications include Cholestatic jaundice, Hepatic necrosis, Lupus like syndrome, Nephrotic syndrome, Insulin autoimmune syndrome.Carbimazole produces potentially life-threatening effects which include Agranulocytosis, Oropharyngeal bacterial infection, which are responsible for the discontinuation of Carbimazole therapy. The symptomatic adverse reactions produced by Carbimazole are more or less tolerable and if they become severe, they can be treated symptomatically, these include Headache, Nausea, Fever, Loss of hair, Arthralgia, Urticarial skin rash, Vasculitis.

Warnings

Excessive dose of carbimazole (antithyroid) should not be used, because it may cause hypothyroidism or goiter Treatment should be discontinued immediately if there is any clinical or laboratory evidence of neutropenia. It is not recommended to use high dose during pregnancy or lactation.

High Risk Groups

Drug should not be given to Paediatrics, Pregnant Mothers, Geriatrics, and Neonates.If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.

Adult Dosage

20 to 60 mg (40 (40)) 8 hourly PO — Initially, As Required 5 to 15 mg (10 (10)) 8 hourly PO — Maintenance, for 6-18 Months

Child Dosage

0.25 mg/kg (0.25 (0.25)) 8 hourly Oral — -

Neonatal Dosage

0.25 mg/kg (0.25 (0.25)) 8 hourly oral —
