Overview
Carnitine is a betaine derivative found in skeletal muscle and liver. Carnitine is required for mitochondrial ß oxidation of fatty acids. Carnitine is involved in energy metabolism. It has been used as an investigational anti-thyroid and anti-angina agent.
Indications
Carnitine is primarily indicated in conditions like Carnitine deficiency, Pediatric cardiomyopathy, Primary carnitine deficiency, Secondary carnitine deficiency, Valproate toxicity, End-stage renal disease.

Side Effects
The symptomatic adverse reactions produced by Carnitine are more or less tolerable and if they become severe, they can be treated symptomatically, these include Nausea, Vomiting, Diarrhea, Abdominal cramps, Hypertension, Dizziness, Fever, Anemia, Paresthesia, Cough, Tachycardia, Vertigo, Palpitation, Edema, Gastritis.
Warnings
Carnitine should be used with caution in patients with pre-existing illnesses or allergies.This medication should be used only if clearly needed during pregnancy or lactation.
High Risk Groups
Drug should not be given to Pregnant Mothers, and Neonates. If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.
Adult Dosage
1 to 3 g/day (2 (2)) As recommended. PO — For solution dosage forms. The dose should be increased slowly while assessing tolerance and therapeutic response.

Drug Interactions
Blood carnitine concentrations may be decreased in children who are concomitantly taking the anticonvulsants carbamazepine, phenobarbital, phenytoin, or valproic acid. Children concomitantly taking pivampicillin and other pivalate-conjugated antibiotics may also have reduced blood carnitine concentration levels.
