Contraindications
Chlorpromazine (HCl) is contraindicated in conditions like Epilepsy,Myasthenia gravis,Narrow-angle glaucoma,Parkinson's disease,Depressed level of conciousness.
Side Effects
WARNING/CAUTION: Even though it may be rare, some people may have very bad and sometimes deadly side effects when taking a drug. Tell your doctor or get medical help right away if you have any of the following signs or symptoms that may be related to a very bad side effect: Signs of an allergic reaction, like rash; hives; itching; red, swollen, blistered, or peeling skin with or without fever; wheezing ; tightness in the chest or throat; trouble breathing, swallowing, or talking; unusual hoarseness; or swelling of the mouth, face, lips, tongue, or throat. Signs of lupus like a rash on the cheeks or other body parts, sunburn easy, muscle or joint pain , chest pain or shortness of breath , or swelling in the arms or legs. Dizziness or passing out. Shakiness, trouble moving around, or stiffness. Drooling. Change in the way you act. Seizures . Swelling in the arms or legs. Change in eyesight. Any unexplained bruising or bleeding. Enlarged breasts or nipple discharge. Ejaculation problems. Not able to get or keep an erection. For women, no period. Trouble passing urine. Feeling very tired or weak. Trouble sleeping. Fast or abnormal heartbeat. Muscle weakness. Restlessness. Yellow skin or eyes. A very bad and sometimes deadly health problem called neuroleptic malignant syndrome (NMS) may happen. Call your doctor right away if you have any fever, muscle cramps or stiffness, dizziness, very bad headache, confusion, change in thinking, fast heartbeat , heartbeat that does not feel normal, or are sweating a lot. Some people may get a severe muscle problem called tardive dyskinesia . This problem may lessen or go away after stopping Thorazine (chlorpromazine injection), but it may not go away. The risk is greater with diabetes and in older adults, especially older women. The risk is greater with longer use or higher doses, but it may also occur after short-term use with low doses. Call your doctor right away if you have trouble controlling body movements or problems with your tongue, face, mouth, or jaw like tongue sticking out, puffing cheeks, mouth puckering, or chewing.
Warnings
Chlorpromazine should be used with caution in patients exposed to extreme heat or phosphorus insecticides, patients in state of alcohol withdrawal, patients with history of peptic ulcer, allergic reactions, with renal impairment, prostatic hypertrophy. Photosensitization may occur, caution patients to take appropriate measures (e.g. wear protective clothing or sunscreen) against exposure to sunlight or ultra violet light. Discontinue use at least 48 hours before myelography.
High Risk Groups
Drug should not be given to Paediatrics, Pregnant Mothers, Geriatrics, and Neonates.If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.
Adult Dosage
25 mg (25 (25)) 8 hourly PO — 25 to 50 mg (38 (37.5)) As recommended. IV,IM — Repeated every 6-8 hours.
Child Dosage
0.5 mg/kg (0.5 (0.5)) 6 hourly Intra Muscular — Should not be used under 6 months of age. 0.5 mg/kg (0.5 (0.5)) 6 hourly Intra Venous — Should not be used under 6 months of age. 0.5 mg/kg (0.5 (0.5)) 6 hourly Oral — Should not be used under 6 months of age.
Neonatal Dosage
0.5 mg/kg (0.5 (0.5)) 4 hourly Intramuscular — 0.5 mg/kg (0.5 (0.5)) 4 hourly Intravenous — 0.5 mg/kg (0.5 (0.5)) 4 hourly oral —
