Cyproheptadine (HCl)

Cyproheptadine is a serotonin and histamine antagonist with anticholinergic and sedative properties, resembles the phenothiazine antihistaminic agents in chemical structure and has potent H1 receptor blocking actions. It is 1st generation Antihistamine

Cyproheptadine (HCl) is primarily indicated in conditions like Acromegaly, Advanced ovarian cancer, Angioedema, Anorexia nervosa, Antidepressant-induced inorgasmia, Appetite stimulation, Carcinoid syndrome, Cushing's syndrome, Hyperprolactinaemia, Migraine, Nelsons syndrome, NSAID associated ulceration, Pruritus, Symptomatic relief of allergy, To prevent recurrence of neural tube defect, Urticaria, Vascular headaches, and can also be given in adjunctive therapy as an alternative drug of choice in Severe nausea, vomiting (prevention).

Cyproheptadine (HCl) is contraindicated in conditions like Asthma, Glaucoma, Prostatism, Pyloric stenosis, Nursing women, Concomitant MAOIs therapy, Stenosing peptic ulcer, Pyloroduodenal obstruction, Bladder neck obstruction, Elderly/Debilitated patients

Adverse reactions which have been reported with the use of antihistamines are as follows:

Cyproheptadine should be used with caution in bone marrow depression. Patients should observe caution while driving or performing other task requiring alertness because it may cause drowsiness. It should be used with caution in patients with urinary retention, history of bronchial asthma, hyperthyroidism, cardiovascular or liver disease. It should be used with caution in person with acute or chronic respiratory impairment, especially in children and elderly patients. Do not administer subcutaneously because irritation or lesion may occur. Elimination of drug is reduce in renal insufficiency. Antihistamines may cause excitation in young children.

Drug should not be given to Pregnant Mothers, Geriatrics, and Neonates.If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.

4 to 8 mg (6 (6)) 8 hourly PO — It may be given 4 times daily. Scraped Dose — Antihistamine overdosage reactions may vary from central nervous system depression to stimulation especially in pediatric patients. Also, atropine-like signs and symptoms (dry mouth; fixed, dilated pupils; flushing, etc.) as well as gastrointestinal symptoms may occur. If vomiting has not occurred spontaneously , the patient should be induced to vomit with syrup of ipecac. If patient is unable to vomit , perform gastric lavage followed by activated charcoal. Isotonic or ½ isotonic saline is the lavage of choice. Precautions against aspiration must be taken especially in infants and children. When life threatening CNS signs and symptoms are present, intravenous physostigmine salicylate may be considered. Dosage and frequency of administration are dependent on age, clinical response, and recurrence after response. (See package circulars for physostigmine products.) Saline cathartics , as milk of magnesia, by osmosis draw water into the bowel and, therefore, are valuable for their action in rapid dilution of bowel content. Stimulants should not be used. Vasopressors may be used to treat hypotension. The oral LD 50 of cyproheptadine is 123 mg/kg, and 295 mg/kg in the mouse and rat, respectively.

MAO inhibitors prolong and intensify the anticholinergic effects of antihistamines. Antihistamines may have additive effects with alcohol and other CNS depressants, e.g., hypnotics, sedatives, tranquilizers, antianxiety agents.
