Overview
Floxacillin (Na) is a penicillinase resistant, semisynthetic penicillin which has been used primarily in the treatment of infections due to benzylpenicillin-resistant staphylococci. Penicillins are active against most bacteria especially gram positive pathogen (streptococci, staphylococci, pneumococci), some negative starins (gonococci, meningoccocci), some spirochetes and some fungi. The structure of flucloxacillin consists of a thiazolidine ring connected to a ß lactam ring, which is attached to a side chain. Antibiotics require constant drug level in body for therapeutic effect. This is achieved by taking the drug at regular interval of time throughout the day and night as prescribed. Floxacillin (Na) is important to take the drug for the full time period as prescribed. If you discontinue the drug it may result in ineffective treatment.
Indications
Floxacillin (Na) is primarily indicated in conditions like Bacterial infections, Cellulitis, Endocarditis, Food allergy, Impetigo, Meningitis, Osteomyelitis, Otitis externa, Respiratory tract infections, Reversal of non-depolarizing neuromuscular blockade, Septicemia, Skin infections, Small cell carcinoma, Soft tissue infections, Staphylococcal enterocolitis, UTI, and can also be given in adjunctive therapy as an alternative drug of choice in Cardiothoracic surgery, Orthopedic surgery.

Side Effects
The severe or irreversible adverse effects of Floxacillin (Na), which give rise to further complications include Cholestatic jaundice, Blood dyscrasias, Neutropenia, Cholestasis, Nephritis. Floxacillin (Na) produces potentially life-threatening effects which include Pseudomembranous colitis, Anaphylaxis, which are responsible for the discontinuation of Floxacillin (Na) therapy. The signs and symptoms that are produced after the acute over dosage of Floxacillin (Na) include Nausea, Vomiting, Diarrhea, Abdominal discomfort. The symptomatic adverse reactions produced by Floxacillin (Na) are more or less tolerable and if they become severe, they can be treated symptomatically, these include GI adverse effects, Phlebitis.
Warnings
Before initiating therapy with flucloxacillin, careful inquiry should be made concerning previous hypersensitivity reactions to beta-lactams. Cross-sensitivity between penicillin and cephalosporins is well documented. Special caution is essential in the newborn because of the risk of hyperbilirubinemia. Studies have shown that, at high dose following parenteral administration, flucloxacillin can displace bilirubin from plasma protein binding sites, and may therefore predispose to kernicterus in a jaundiced baby. In addition, special caution is essential in the newborn because of the potential for high serum levels of flucloxacillin due to a reduced rate of renal excretion.
High Risk Groups
Drug should not be given to Pregnant Mothers, and patients suffering from Kidney dysfunction. If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.
Adult Dosage
125 to 250 mg/kg (190 (187.5)) 6 hourly Inhalation — Desolved in 3 mL of sterile water and inhaled by nebuliser.
250 mg (250 (250)) 6 hourly PO —
250 to 1000 mg (620 (625)) 6 hourly IV — Slow IV for 3-4 mins or by IV infusion.
Child Dosage
0 to 6.25 mg (3.1 (3.125)) 4 hourly Oral — Under 2 Years
12.5 to 0 mg (6.2 (6.25)) 4 hourly Slow IV — (2-10 Years)
Neonatal Dosage
12.5 mg/kg (12 (12.5)) 6 hourly Slow Intavenous —
7.81 mg/kg (7.8 (7.81)) 6 hourly Oral —
