Overview
Methoxamine (HCl) is a direct-acting a-1 receptor agonist. It may cause a prolonged increase in blood pressure via its vasoconstricting action. Methoxamine (HCl) is used parenterally , but clinical applications are rare and limited to hypotensive states, notably in general and spinal anaesthesia.
Indications
Methoxamine is primarily indicated in conditions like Acute hypotension during epidural anesthesia, Acute hypotension during spinal anesthesia, Congenital heart disease, Hypotension associated with cardiopulmonary bypass, Hypotension during inhalational anesthesia, Hypotension induction, Hypotensive anaesthesia, Paroxysmal supraventricular tachycardia.
Contraindications
Methoxamine is contraindicated in conditions like Hypertension, Hyperthyroidism, Myocardial infarction, Coronary artery disease.
Side Effects
The severe or irreversible adverse effects of Methoxamine, which give rise to further complications include Bradycardia, Decreased plasma volume. Methoxamine produces potentially life-threatening effects which include Heart failure, Severe hypertension, Cerebrovascular accidents. which are responsible for the discontinuation of Methoxamine therapy. The signs and symptoms that are produced after the acute overdosage of Methoxamine include Bradycardia, Headache, Chest pain, Decreased blood flow to vital organs. The symptomatic adverse reactions produced by Methoxamine are more or less tolerable and if they become severe, they can be treated symptomatically, these include Headache, Nausea, Vomiting, Anxiety, Sweating, Micturition, Piloerection, Hypertension, Vasoconstriction.
Warnings
Use with caution in patients with ventricular fibrillation. Administer with great caution and in carefully circumscribed quantities in area of body served by end arteries (e.g. fingers, toes etc.). Cautions should be taken in pregnancy, children, sulfite sensitivity.
High Risk Groups
Drug should not be given to Paediatrics, Pregnant Mothers, Geriatrics, and Neonates. If prescribing authority justifies the benefits of the drug against the possible damages he/she should reevaluate them and consult the reference material and previous studies.
Adult Dosage
10 to 15 mg (12 (12.5)) As recommended. IM — Should be given shortly before or at the time of administering spinal anesthesia to prevent a fall in blood pressure.
Child Dosage
250 ug (250 (250)) As recommended. IM — 80 ug/kg (80 (80)) As recommended. IV —

Drug Interactions

The mechanism is an additive or synergistic vasoconstriction due to the alpha adrenergic agonist activity of ergot alkaloids. ModerateThe use of dihydroergotamine injection or nasal spray with vasoconstrictive agents is not recommended and other ergot alkaloids should be administered with caution. Clinical monitoring of patients response and tolerance, including measurements of vital signs and monitoring for excessive vasoconstriction, is recommended if these products are used in combination. Patients should be advised to notify their doctor if they experience numbness, tingling, or cyanosis in the extremities, muscle pains, weakness, or chest pain or tightness. These interactions are sometimes beneficial and sometimes may pose threats to life. Always consult your physician for the change of dose regimen or an alternative drug of choice that may strictly be required.
