Overview
Remifentanil is an opioid analgesic similar to fentanyl, used as an adjunct to general anaesthesia. Remifentanil is given as an induction agent since it has a rapid onset of action and an extremely short duration of action. Remifentanil is metabolized very rapidly by blood and non-specific tissue esterases, making its pharmacokinetic and pharmacodynamic half-lives extremely short.
Indications
Remifentanil is primarily indicated in conditions like Anesthesia induction, Anesthesia maintenance, Conscious analgesia, Analgesia immediate post-op period.
Contraindications
Remifentanil is contraindicated in conditions like Phaeochromocytoma, Raised intracranial pressure, Adjunct in cryptococcal meningitis.
Side Effects
The symptomatic adverse reactions produced by Remifentanil are more or less tolerable and if they become severe, they can be treated symptomatically, these include Drowsiness, Nausea, Vomiting, Constipation, Respiratory depression, Bradycardia, Hypotension/hypertension, Shivering, Apnea, Hypoxia, Biliary tract disease.
Warnings
Skilled activities such as driving should be avoided because of drowsiness.
