How to Fill Request Form (for Sending Specimen)

This protocol was migrated from the earlier Pharmapedia and Ward Guide apps for educational use. Follow your hospital's own policies and consult seniors when in doubt.
- Patient's names, age, gender, registration number, ward name, and hospital names are written.
- Brief clinical details of the patients are described; operative findings are mentioned and clinical impression of the case is given.
- Finally, the Doctor's (sending the specimen for pathology) name and designation along with time and date are written.
- Always check with the consultant, where he/she wants the biopsy to be sent and, are there any special instructions that he/she wants to give about this particular biopsy specimen.
- Biopsy jar /container must clearly mention if it contains hazardous material (e.g. radioactive; biohazard- patient is HIV positive; careful when slicing as the specimen contains metal clips, etc.
